Homeschooling Report Track for Infants and Toddlers

I created a quick survey / report tracker for my toddler in order to keep our days organized. My toddler needed structure since he no longer attended CSU’s Child Development Center Program. We used to receive daily reports on his activities and milestones. Now that he is home, I have had to create a weekly syllabus, buy materials, and organize his days so that he is not watching TV all day.
I built the assessment survey as a tracker to better inform me on my toddler’s improvement and challenges in regards to his social, emotional, intellectual, and physical development. It’s not perfect but it is a start. I will be developing and improving the survey questions on a weekly basis. During this COVID-19 shutdown we have been forced to experiment and create reliable resources for our children, and I am just sharing what I have developed thus far. Feel free to copy / paste and modify your report to your children’s environment and needs.
